Do you see your name on the list? Then claim your prize now!
Follow these instructions:
1. You may claim your prize at Summit Media, Level 1 Robinsons Galleria Mall, EDSA corner Ortigas, Quezon City.
2. Claiming is from Mondays to Fridays, strictly 1:00 to 5:00 p.m. only (except holidays).
3. Bring a photocopy of your valid I.D. If someone else is claiming for you, prepare an authorization letter.
We will not release your prize unless you have these requirements.
4. Deadline for claiming is on May 30, 2008. Unclaimed prizes beyond the deadline shall be forfeited.
5. No need to call us to set an appointment, just make sure you come within the claiming schedules above.
